A groundbreaking clinical trial has demonstrated that a medication containing tetrahydrocannabinol (THC) significantly reduces the frequency and intensity of nightmares in individuals suffering from post-traumatic stress disorder (PTSD). These pivotal findings were published in the journal "Nature" on August 5, 2026.

The study was conducted by a team of researchers from leading institutions, including Charité University Hospital in Berlin, the University Medical Center Hamburg-Eppendorf, and the Central Institute of Mental Health in Mannheim. A total of 171 adults with PTSD and recurring nightmares participated in this multicenter, double-blind, placebo-controlled trial. The average age of participants was 37.9 years, with a predominant representation of women (79.3%).

Participants were randomly divided into two groups, with one group receiving dronabinol (a prescription medication containing THC) and the other receiving a placebo. What Were the Study Results?

Researchers measured the frequency and intensity of nightmares using an eight-item subscale of the Clinician-Administered PTSD Scale (CAPS). After ten weeks, the dronabinol group reported an average decrease of 3.7 points in nightmare scores, while the placebo group experienced a 2.2-point reduction. This 1.5-point difference was statistically significant and indicated a moderate-to-large effect size.

More than one-third of patients using dronabinol experienced a complete cessation of nightmares by the end of treatment, while an additional 21% reported at least a 50% reduction in nightmare frequency. Furthermore, approximately five out of six participants noted an improvement in their overall health.

How Does THC Affect PTSD Dreams?

The researchers noted that while dronabinol effectively reduced nightmares and improved sleep quality, it did not demonstrate significant effects on overall PTSD severity or depressive symptoms compared to the placebo. THC's interaction with the endocannabinoid system plays a crucial role in regulating sleep, stress, and emotional memory processing, which may explain its impact on nightmare frequency by potentially reducing dream activity during REM sleep.

What Are the Side Effects and Safety Considerations?

Adverse effects were noted in 89.7% of patients in the dronabinol group, compared to 77.1% for the placebo group. Common mild to moderate side effects included dizziness, headaches, and increased appetite. Five patients (5.7%) in the dronabinol group discontinued treatment due to adverse events, while six patients (7.2%) in the placebo group also stopped treatment. Although some serious adverse events were reported in seven individuals receiving dronabinol, no severe side effects were attributed to the medication. Additionally, researchers found no evidence of withdrawal symptoms after completing therapy.

The authors of the study stress that these findings provide valuable evidence that dronabinol may offer relief from PTSD-related nightmares.
